Application For Planned Educational Leave Program (PELP)
The Planned Educational Leave Program (PELP) allows any continuing registered student-- undergraduate or graduate-- to temporarily suspend academic work.

Change Of Major
Used to declare a major, change your major, or declare multiple majors
Conditions: Active undergraduate student

Declaration Of A.B. Or B.S. Degree Type
If you are a student in certain majors, you need to indicate whether you are pursuing a Bachelor of Arts or Bachelor of Science degree.
Conditions: Major in Anthropology, Biological Sciences, Chemistry, EEB (Evolution, Ecology, and Biodiversity), Exercise Biology, Geology, Mathematics, Microbiology, Physics, Plant Biology, Psychology, or Statistics

Excess Units Petition
Used to justify the need for enrollment beyond 200 units to complete your degree
Conditions: Not needed by students in Engineering

Extend Incomplete Deadline
Used to extend the three term deadline for completing courses with an I grade
Conditions: Can only be submitted by students with an incomplete course

Used by students to indicate they plan on completing a minor program. An approved Minor Declaration does not indicate a minor will be awarded.

Multiple Repeat Petition
Used by students wishing to repeat a course for a second or subsequent time. This petition must be submitted and approved prior to enrollment.

Petition For General Appeal
A Petition for General Appeal may only be submitted for processes or deadlines governed by the Office of the University Registrar.

Petition For Name Change On University Record
This form allows you to change your name on all of your university academic records after a legal name change.

Petition For Part-Time Status
Part-Time allows for a fee discount based on a reduced course load. It also waives the minimum progress requirements.

Senior Residence Waiver
Used by students wishing to complete more than 10 of their final 45 units at another institution. Must have at least 145 units to submit.
Conditions: Undergraduate students with 145 units or more
